For me it was The Omen, for story, acting and dialogue. The Descent and Carrie for surprise and Saw for cringe moments.

i would say In the Mouth of Madness is one of my favorite stories in horror, even when it made little to no sence. For scariness it would have to be probably a Dario Argento movie where they use that piano wire machine that the killer would stick between things and cut off peoples heads. I forgot the name.
